6、Application Form: Specific Additional Guidance for Application Form: Personal Information: 1. 请严格按照护照上的姓名拼音填写，并请注意姓名的正确填写顺序： First Name 为名 字 ， Family Name为姓。 三个字或三个 字 以上的名字 请勿将 名字 拆开填写在 Middle栏目中 ， 请 将名字 全部填写在 first name 处。 入选后所有的文件姓名拼写都将以此为准。 2. 申请表上的 出 生地（ Place of birth） 请 与护照上的出生地（ Place of birt

7、h） 保持 一致 ， 尽管 护照上 的出生地一般指的是省份 。 大家就 填省份的信息即 可。因为入境美国时， 富布赖特 项目颁发的 所有 证明您身份的 文件，特别是 DS2019 表的信息必须与护照上的信息一致。 Academic Credentials: 1. Institution Name, 请填写您所在大学的名称， 而不是研究所或学院的名字。 2. Brief Summary of project Statement/Rationale: 研究课题摘要 不得超过 700 个 词 ， 并只能使用 新 罗马体字 （ Times New Roman） ， 请 不 要 使用回车键 ， 否则系统将无法显示您所填写的内容。 Project Details: Proposed Program Length a

10、nd Dates: 富布赖特项目 是十个月的交流项目。 所以 项目长度 应该 填写 10 个月。 但申请人根据课题需要 可以选择 12 个月 作为您的富氏项目期限 。 无论选择 10 个月或 12 个月 作为项目期限 ， 不会影响 您的 资助金额总额。延长的两个月生活费需要自行承担，项目不提 供由延期引起的 额外津贴补助。 富氏 学者 每年 不得 早于 九月 或晚于同年 十二月 开始 项目。 项目 起始日请 根据自己的实际 需求来填写， 比如可填 2016 年 9 月 16 日 等。 但是对于计划带学龄孩子去美国上学的学者，建议 选择 9 月 1 日 作为项目起始 日， 以便您有足够时间为孩子

11、办理入学手续。您可以在项目开始的前 30 天入境美国 安排 家属 生活 或上学 有关的事宜。 Project Details： Major Academic Discipline 请学者务必从下拉菜单里选择自己的学科 领域 。 Professional Information： Professional travel and/or residence abroad during the last five years (list most recent first; time abroad exceeding 3 months) If you have entered the United S

If you have not made

17、会影响 您的 申请结果。 如果是计划自己联系 美国的接待院校 ，您可以 在 申请阶段 就开始 联系 美国教授 ， 以便早些获得一封美国大学 接收函 。但是如果您 到了 提交申请材料 的 最后期限时还没获得邀请函， 也 不必紧张。在面试时 不要求提供 邀请信 ， 只要 能够陈述清楚 在申请表上 所列举的三所学些的原因。 无论 选择 CIES 还是自己 联系 美国 接待院校 ， 请务必填 写完整大学、院 系和相关教授的信息。否则将 延误 您的美国接待 学校 安置 。 申请人务必填按照 优先顺序将三所（ 最多 只能 写 三所）感兴趣 的 美国大学填写在所给出的地方。 如果您 在填表时已经收到了美国大

18、学的邀请函， 可以 只 填 写 收 到邀请函的 那 所学校， 并将邀请函上传至 Embark 系统。 Contact/Family Information Telephone: 请务必填写 手机号码 ，而不是座机电话以便项目管理人员与您联系 Emergency Contact Information： Telephone: 请务必 填写手机号码。 建议 填写一 位在美的亲朋好友， 一位在国内的联系人的信息。 Accompanying Dependents: question 30 该项目允许 携带 家属 。 如果您现在无法确定家属去美国 的期限， 请先填写一个大概的时间段，但家属的其它信息必须

19、准确无误。请只填写配偶和岁以下未婚孩子的信息。按照项目管理规定父母 及兄弟姐妹不能作为富布赖特家属身份前往美国， 而只能 以旅游者身份入境美国。 如果家属 陪伴 学者在美国的时间超过个月，学者可以享受每月美元的家属补贴 (两人及 两人 以上为每月 350 美元 ) 用于支付家属购买保险 的部分 费用。家属在美的生活费用 自理。 家属是美国 公民者，请持美国护照入境美国，无需办理签证 入境美国 。 Alternate Funding: Please list all non-Fulbright funding you expect to receive during your grant (sa

37、 的 。 Financial Support/Budget 您 在申请阶段 可暂 不提交此证明 。入选后 选择 12 个月期限的学者 需要出示银行流水账或存款证明（ $2500/per person/per month）以证明你有能力承担您和您的随行家属在美 2 个月的生活费用。何时提交 资金证明 入选后 会另行通知 。 J-1 or J-2 VISA (DS-2019) 如果您 和您的家属 曾 经 持 J-1 或 J-2 交换学者签证入境 美国， 请将 使用过的您和您的家属 DS 2019 表的复印件上传至此 。 超过五年的 DS2019 表 就不用 提交了。 Passport 在申请 阶段

38、 请只 上传 申请人本人的护照 复印件， 家属可以选择入选后在提交。具体时间会另行通知。 Bibliography 请上传一份 -页的与您的富布赖特研究课题有关的参考书目。如果有中文参考书籍，请务必将书名翻译成英文。 Letter of Support from Home Institution (if applicable) 如果学校给您支持信，可以上传至此， 但不是必须。 支持信 必须使用英文。 Step 4: References: 提交短文后，请点击如图所示按钮 4 references，注册三名推荐人的信息。注册成功后您的推荐人将自动收到一个登陆富布赖特网上申请系统的邮件和密码 ，您的推荐人就可开始在网上为您填写推荐信（回答六个问题）。
