8、ion 备注：此单一式四份，售票单位一份，始发站值机单位一份，旅客一份，到达站小动物交付工作人员一份。北京首都航空有限公司小动物运输申请书（背面）APPLICATION FOR PETS TRANSPORTION OF CAPITAL AIRLINES一小动物是指家庭驯养的小狗、猫。二小动物运输限制和条件1、提前48小时向首都航空售票单位进行申请。2、除导盲犬、助听犬及救助犬等工作犬外均不能作为客舱行李运输，必 须办理托运。3、每名旅客限制仅能运输一只小动物。4、小动物作为托运行李运输时， 宠物和笼子的总重量重量不得超过20Kg。5、导盲犬、助听犬、救助犬等工作犬运输不受本条3及4款规定限制。
9、6、对于出现的以下情况，承运人无法 办理航空运输： 托运人所托运的活体动物为国家禁运的动物之列。 具有传染病征候,或疑似传染病载体的动物(如发生地区禽流感的家禽)。犬类动物及猫科动物年龄小于六周。 怀孕动物或是在飞机起飞前48小时之内刚刚分娩过的动物。 需要托运人完成的动物飞行运输准备未完成。运输过程中可能产生的承运人认为的天气、高热（气温通常高于85F/29C 的往返机场）等不适于哈巴狗或扁 平鼻的狗或猫运输的其他原因。因航班机型原因,不适于运输活体动物。（如货舱无适运舱位） 不符合始发、经停、目的地城市或国家 动物运输的相关规定航空公司工作人 员判断该动物有可能 对旅客和/或其行李造成损害
10、，或者航空运输有可能对该动物造成损害。三、运输文件要求1、乘坐国际航班，旅客应在至少出发前 7 天为宠物备妥下列文件相关政府（目的地 /中转地）核发的有效输出/ 输入文件。有关当局核发的有效健康声明 书及狂犬病疫苗注射 证明书。 妥善的备齐所有行程中含盖的入境/过境国要求的入境许可、健康声明书及疫苗注射证明。任何行程中含盖的入境 /过境国政府要求的额外特殊文件2、国内运输所须文件动物卫生监督所出具的动物检疫合格证明，其中“ 运载工具消毒情况”一栏必须填写且单据上需盖有动物卫生监督所检疫专用章。3、导盲犬、助听犬、救助犬等工作犬运输所须文件：除须备齐本条1或2款证件外，还应携带有效的动物训练合格
11、证明书、动物工作证或/和动物身份证。四、宠物箱要求1、首都航空建议旅客使用专用航空箱运输动物。2、宠物箱必须由坚固材料制成且顶部固定。乘坐国内航班至少三面通风，国际航班四面通风，且 门必须由金属制成。硬件（包括螺母、螺丝以及门）确保安装完整。箱门牢固须有锁闭装置，有固定的把手或凸起 边缘。 宠物箱的空间应足够大，可以保证宠物自由站立或坐下、转身和以正常姿势躺卧。宠物箱的底部平稳，能够固定在平整的面上而不滑动。 带轮子的宠物箱，需要进行必要的固定。保证在运输过程中轮子不滑动.宠物箱必须经过打包处理，中号箱每面至少捆扎 2匝，呈“井” 字形，大号箱每面至少捆扎 3 匝，打包带分布均匀，保 证在运输
12、过程中正常情况下打包带不脱落。3、宠物箱箱内须铺上吸水性衬垫，例如毛巾、毯子或者白色的纸，以防止宠物排泄物外溢污染其他行李。北京首都航空有限公司1、Scope of The Pets Transportationpets only limit to the domestic dogs，cats . 2、Limitations and Conditions of The Transportation (1)The passengers have to apply to the ticket agency of Capital Airlines at least 48 hours prior to
13、 departure.(2)Each passengers can only have one pet checked.(3)Except the service animals, such as the seeing-eye dogs, hearing-ear dogs and the rescue dogs, all the other pets are forbidden to be carried in the cabin, they must be transported as checked baggage.(4)Pets transported as checked baggag
14、e, the total weight of the pets and container cannot exceed 20Kgs.(5) The service animals are exempt from the limitations of the item3 and 4.(6) Pets may be refused transportation in case of the following situations.The checked pets belong to the forbidden transporting animals of the nation.pets wit
15、h communicable disease symptoms or suspected communicable carrier, such as the poultry with regional bird fluThe age of dogs and the cats is less than 6 weeks.The pregnant pets and the pets that have just given birth within 48 hours before the departure of the aircraft.the preparation including trav
16、el documents, container are not completed before the transportation.Temperature restrictions may apply when the expected airport temperature is higher than 85f/29c（flat nose dog or catcant be transported）.The aircraft type is unfit for transporting the live animals.( For example, if the aircraft hol

It is imperative that passengers consult with Staff before the transportation.)The pet doesnt conform to the relevant regulations of the destination city.The staff judge that the checked pets may hurt other passengers and/or their baggage. The pets may be
18、 hurt during the transportation.3、The Necessary Documents for Transporting Pets1) passengers should prepare the following documents below at least 7 days before the departure for international flight.Valid “Certification for animals quarantine“，especially the “vehicle disinfection situation“ must be
19、 filled and stamped with the official department.2) Documents for the domestic transportationValid Animals Immunity Certificate which should contain at least the rabies vaccine certificateValid Quarantine Certificate for pets issued by China government.Valid Quarantine Certificate for container issu
20、ed by China government.3) The documents for service animalsBesides the certificates mentioned above in the item 1 and item 2, the passengers should also carry a valid Qualified Certificate for Pets Training, Employees Card of The Animals and/or the I.D. card. 4、The Requirements Of The Container.Capi
21、tal Airlines suggests passengers use the special shipping container designed for transporting pets by air. The container must have adequate ventilation with at minimum three ventilations ports for domestic flights and four ventilation ports for international flightsHardware(nuts/bolts/screws)complet
22、e and assemble.Door is locked and is secured with releasable ties.Kennel sizes allows animals to stand and sit erect, turn around and lay.The bottom should be stable,Fixed on a flat surface and will not slip.The container with wheels, need to undertake necessary fixed ensure that cant be sliped duri
